Freedom of Information – Publication Scheme

Manor House Surgery is a public authority for the purposes of the Freedom of Information Act 2000 in relation to information held about NHS primary medical services.

We have adopted the Information Commissioner’s Office (ICO) Model Publication Scheme, which commits us to making information routinely available to the public.


ICO Model Publication Scheme (Unedited)

This model publication scheme has been prepared and approved by the Information Commissioner.
It may be adopted without modification by any public authority without further approval and will be valid until further notice.

This publication scheme commits Manor House Surgery to make information available to the public as part of its normal business activities.

The scheme commits the practice to:

  • Proactively publish or otherwise make available information it holds
  • Specify the information available within each class
  • Publish how information can be accessed
  • Regularly review and update published information
  • Publish any applicable datasets in line with Freedom of Information requirements

  • Total NHS income
  • High‑level financial information relating to NHS services

How available: On request
Charge: Free (reasonable copying costs may apply)


Class 3 – What our priorities are and how we are doing

Information available:

  • Practice development priorities relating to NHS services
  • CQC inspection reports

How available: Website or on request
Charge: Free


Class 4 – How we make decisions

Information available:

  • Governance and management arrangements
  • Decision‑making processes affecting NHS service delivery

How available: On request

Class 5 – Our policies and procedures

Information available:

  • Freedom of Information Policy
  • Complaints Policy
  • Data Protection & Confidentiality Policies
  • Health & Safety Policy

  • Statutory registers where applicable

Important notes

  • Information relating to individual patients or staff personal data is not available under FOIA and must be requested under UK GDPR
  • Some information may be exempt from disclosure
  • Charges are applied only where permitted by FOIA regulations
